* Additional Services include meetings or telephone calls with schools or other professionals involved in the participant's care and resource creation.  These fees will be discussed and agreed with you before being charged.

​**Off-site appointments are limited to a 30 minute drive from the practice.

Service Agreements

Your clinician will work with you to devise a treatment plan to help you to work towards your therapy goals.  

As part of this process you will make a Service Agreement during your first appointment to say how often you will attend appointments and whether you would like to include off-site visits (eg to school or kindergarten) and plan review reports in your Service Agreement.  

This will ensure that you and your clinician are clear on what the plan is and how much of your budget will be reserved for your supports from Hopscotch and Harmony.

You will be provided with a copy of your Service Agreements for your records.
